Family Alliance receives grant from Community Foundation

WOODSTOCK – Family Alliance will get $25,000 from the McHenry County Community Foundation to put toward an expansion of its adult primary care services.

The money will help Family Alliance integrate primary care into its existing services to create a "team-based, patient centered home medical model that can optimize treatment plans," Executive Director Kim Larson said in a news release.

Family Alliance has received funding toward primary care from other organizations, including the Russell and Josephine Kott Memorial Trust, the Willow Springs Foundation, the Alfred Bersted Foundation, the Fred K. Brunner Foundation, and the Aptargroup Charitable Foundation, the release said.

Dr. Rex Nzeribe sees patients through the primary care program on Mondays, Tuesdays, Wednesdays and Fridays at the Family Alliance Woodstock facility, 2028 N. Seminary Ave.
